A study by the Pan African Institute for Health and Clinical Evidence (PAHOce) found that the services market was dominated by established physiotherapy services companies, despite new developments in the market, particularly for the elderly population. The report defined four important sectors involved in the physiotherapy services market and how these sectors were influencing the market and how the market was influencing them: rural area, ethnicity, family income and age. Rural area was found to be the most important barrier in availing physiotherapy services to the poor in rural communities in Nigeria. The barriers include: physical disabilities, socio-economic disadvantages, language and cultural barriers, healthcare system complexity and management systems.
As per the POHce's report, older persons, women and rural health workers were the least likely to have access to optimal physiotherapy services. There was some increase in the number of poor knowledge providers but not enough to change the overall situation. The barrier was further explained as: physical disabilities, language and culture barriers, healthcare system complexity and organizational challenges. In this article, we shall see how poor knowledge and disability in rural health workers can be addressed with improved quality of care, improved accessibility, optimal services and optimized outcomes. The POHce's research team visited 14 rural health facilities across the state of Lagos. It was found that there were inadequate numbers of therapists available for service provision, particularly for the mentally and physically challenged persons. For the disabled population, the numbers of trained and certified therapists was inadequate and the number of service users was increasing at an alarming rate. During the visit, a detailed study area was prepared and the problems discussed with the project manager. Outcomes of the study area were then compared with the national standard benchmarks against which the sample size was calculated.
A qualitative study was then done using the same sample size and identified gaps in the provision of physiotherapy services. The gaps identified in this study were then discussed with the service provider. The results showed that lack of availability of competent physiotherapists and the time taken by healthcare workers to appropriately diagnose and treat these patients, was one of the biggest barriers to access. This was also confirmed when the staff time for providing physiotherapy services was analysed.
Based on the study, several recommendations were made to address these critical issues and challenges.
The major recommendations included recruiting more and professionally qualified physiotherapists, training health care workers in the diagnosis and treatment of physical therapy and developing an infrastructure to provide the necessary inpatient and outpatient care. It is very important to note that the physiotherapy services market in the United Kingdom is very fragmented and many areas of the country are experiencing issues in recruitment and coordination of health professionals. It will be very important to address these gaps to ensure effective and convenient provision of physiotherapy services to the elderly and physically disabled community in the UK.
